Bulimia y Anorexia



ANOREXIA

Las personas que padecen de anorexia tienen una imagen distorsionada de su cuerpo ( se ven gordos, aun cuando presentan un estado de extrema delgadez.).
La anorexia nerviosa consiste en una alteración grave de la conducta alimentaría que se caracteriza por el rechazo a mantener el peso corporal en los valores mínimos normales, miedo intenso a ganar peso y una alteración de la percepción del cuerpo.

Generalmente la pérdida de peso se consigue mediante una disminución de la ingesta total de alimentos. Aunque los anoréxicos empiezan por excluir de su dieta todos los alimentos con alto contenido calórico, la mayoría acaba con una dieta muy restringida, limitada a unos pocos alimentos, acompañado  muchas veces por ejercicio físico excesivo.


 




*   Causas
Actualmente no existe una causa única para la anorexia nerviosa, aunque las investigaciones han arrojado ciertas pistas en el campo médico y psicológico. Al igual que en la bulimia, las causas de la anorexia son múltiples y difíciles de valorar. Todas ellas, tantos las individuales como las familiares, sociales y culturales, deben tenerse en cuenta de cara a un tratamiento. Hay muchos enfoques distintos pero todos ellos señalan dos fases en el tratamiento de este problema: como primer paso debe recuperarse el peso a través de una realimentación controlada médicamente. La recuperación física trae consigo una mejora en algunos aspectos psicológicos como la percepción de la imagen corporal o la obsesión por el peso. Una vez que el estado físico ha mejorado, el tratamiento se centra en los pensamientos, sentimientos y conductas que resultan poco adaptativos. Se trata de mejorar la autoestima y de estimular nuevas formas de expresar sentimientos y valorarse a sí mismo, reconciliando a la persona con su cuerpo y sus necesidades.
Algunos expertos creen que esta enfermedad se origina en las altas demandas de la familia y la sociedad: el ciclo destructivo comienza con la presión que el individuo siente por ser delgado y atractivo. El problema se centra, entonces, en una baja autoestima.
Para otros investigadores, la anorexia nace a raíz de otro problema. Este tipo de desorden podría desarrollarse en cierto tipo de familias disfuncionales, ya observadas anteriormente en casos de pacientes anoréxicos. En ellas, los miembros se vuelven tan interdependientes que no pueden alcanzar su identidad como seres individuales. Parte de esta disfunción se traduce en un miedo a crecer por parte de los niños de la familia. Entonces, especialmente las niñas, comienzan una dieta para evitar que sus cuerpos se desarrollen.




CONSECUENCIAS FÍSICAS

Corazones pequeños:

Niñas de 17 años con corazones del tamaño de una de siete. Quedarse, literalmente, en los huesos está provocando alteraciones en el funcionamiento y en el tamaño del corazón. Los expertos desconocen aún si la recuperación del peso devolverá la normalidad al funcionamiento cardiaco.



Niñas menopáusicas

Bulimia nerviosa

La palabra bulimia significa hambre de buey y procede del griego boulimos (bous: buey; limos: hambre). Para las personas con bulimia, que afecta diez veces más a las mujeres que a los hombres, la comida es una adicción placentera y autodestructiva.

Es un desorden alimenticio. Esta enfermedad se caracteriza por episodios secretos de excesiva ingestión de alimentos, seguidos por métodos inapropiados para controlar el peso como el vómito autoinducido, el abuso de laxantes o diuréticos y la realización de ejercicios demasiado exigentes para el cuerpo. En un 70% de los casos este trastorno acompañado de anorexia (bulimarexia) y en un 30% se manifiesta como bulimia pura . La principal diferencia radica en que en la bulimarexia no sólo no hay adicción a los alimentos, sino que hay un rechazo expreso, intercalado de atracones esporádicos. El acceso de bulimia o atracón se inicia con una sensación de hambre voraz e incontenible, con preferencia por los dulces y otros alimentos de alto valor calórico. La frecuencia de los atracones es muy variada y los enfermos pueden darse varios en el mismo día durante varios días seguidos. Con los atracones suelen sobrepasar las 5.000 calorías diarias. Hay bulímicos que han llegado a superar en un día las 25.000 y alguno ha muerto por dilatación aguda de estómago. La bulimia está considerada como una enfermedad invisible porque puede pasar mucho tiempo sin que el entorno del enfermo la perciba.



*   Causas

Al igual que la anorexia, no se ha logrado descubrir una causa orgánica definitiva para la bulimia. Su proceso de gestación comienza cuando existe algún grado de insatisfacción con la apariencia del cuerpo. En este caso, el individuo siempre se considerará con exceso de peso, aunque la realidad sea otra. Así, comenzará a hacer dieta y, viendo que la imagen en el espejo no cambia, la dieta se intensificará hasta llegar a las prácticas bulímicas.

Entre las causas de este problema se encuentran las experiencias de rechazo social o de fracaso que se atribuyen al peso y los consejos de las amigas. La constatación de que en nuestra cultura la delgadez se considera un requisito para el éxito lleva a querer perder peso y empezar dietas estrictas que no pueden seguirse y nos hacen sentir no sólo fracasados, sino también hambrientos. Las dietas se rompen con atracones y la culpabilidad por las calorías consumidas y la posibilidad de engordar llevan al vómito.

Algunos expertos creen que esta enfermedad se origina en las altas demandas de la familia y la sociedad: el ciclo destructivo comienza con la presión que el individuo siente por ser delgado y atractivo. El problema se centra, entonces, en una baja autoestima.

El trastorno puede aparecer a cualquier edad y repetirse en diferentes momentos de la vida, generalmente inducido por acontecimientos vitales traumáticos o negativos separación de la familia para estudiar la carrera; la segunda crisis, divorcio, perdida de un ser querido.




CONSECUENCIAS FÍSICAS

 
La bulimia es una enfermedad grave que 
puede producir serias complicaciones médicas:Alteraciones digestivas: colon irritable, megacolon, reflujo gastrointestinal, hernia hiatal, perforación esofágica, dilatación y rotura gástrica, pancreatitis.




* Alteraciones metabólicas: descenso de glucosa, cloro, calcio y potasio en sangre. Deshidratación.

* Alteraciones cardíacas: prolapso de la válvula mitral y arritmia, con riesgo de muerte.

Otras alteraciones pueden ser del tipo genitales, óseas y renales.


La mortalidad es superior a la de la anorexia, falleciendo por complicaciones médicas derivadas de los "atracones", vómitos y uso de laxantes, o por suicidio.
